Emergency First Aid Training Registration Form
This day long course is offered as part of the First Aid Program to all faculty and staff designated to be First Aid Attendants, and those who wish to gain a general knowledge of first aid principles and emergency treatment of injuries.
Space is limited, and registrations are accepted as per Laurier's First Aid Procedures.
Priority acceptance will be given to Staff and Faculty who have been designated as a first aid attendant for their area. First aid attendants are responsible for administering first aid treatment and managing a first aid station. SHERM may require confirmation of your role as first attendant from your supervisor.
Priority acceptance will also be given to registrants when first aid training is appropriate based on the risks involved with their position or department/faculty. SHERM may require confirmation of the appropriateness of this training from your supervisor.
If space remains available, registrations will be accepted for Staff and Faculty who do not manage a first aid station.
Cancellation Policy: Course cancellations must be made via email to firstname.lastname@example.org THREE WORKING DAYS prior to the course date. Departments will be invoiced the course cost of $100 per registrant for failing to attend without specified notice. For courses canceled at the discretion of SHERM, a two working day notice will be provided to participants, with the exception of situations involving circumstances beyond SHERM's control.
NOTE: Students interested in taking this course may be accommodated if space permits. Students must register for this course by contacting email@example.com. A $100 fee is required to attend this session. Cheques must be brought to the SHERM office two weeks prior to the course, and can be made out to Wilfrid Laurier University.
